Two template tools are available for hospitals to use or customize to document ABO verification of transplant recipients and living donors in the operating room. The tools were developed by the federal Center for Medicare and Medicaid Services (CMS) and the United Network for Organ Sharing (UNOS), which operates the Organ Procurement and Transplantation Network (OPTN) under federal contract with the Health Resources and Services Administration (HRSA).
The templates provide information about CMS and OPTN documentation requirements to assist hospitals in developing their own processes. Hospitals may use the template tools or customize them to meet the needs of their individual programs:

Recipient Operating Room Verification template
Under OPTN policies and CMS regulations, hospital staff must document verification that the correct organ is transplanted into the correct recipient, and that blood type and other vital data are compatible with the intended candidate immediately before removal of a living donor’s organ(s), and, if applicable, prior to removal of the recipient’s organ(s).
