ChristianaCare is seeking a Renal Transplant Coordinator

The Kidney Transplant Program Team at ChristianaCare is looking Renal Transplant Coordinator to join the team. The Kidney Transplant Program at ChristianaCare offers the only adult kidney transplant program in Delaware.  More than 1,000 patients rely on our transplant team for kidney failure care.  The care provided encompasses kidney transplant evaluation, waitlist care, after transplant care and transplant education, as well as living donation.  The team is multidisciplinary in nature, including transplant surgeons, transplant nephrologists, nurses, social workers, dietitians, pharmacists, financial coordinators, medical assistants and office support.

What you will be responsible for
Identify transplant candidates upon referral and interact with appropriate staff to initiate evaluation process.
Coordinate evaluation as well as pre and post-transplant care with other team members including but not limited to social worker, pharmacist, dietitian and financial coordinator.
Act as a liaison between the transplant program and dialysis and / or nephrology facilities
Assure performance of necessary tests and studies to determine patients’ candidacy for transplant.
Monitor and follow all pre and post-transplant diagnostic studies.
Evaluate patients’ health status on a regular basis.
Assume liaison role between patients’ family and other members of the health care team.
Assume a resource role regarding transplantation for all inpatient staff nurses.
Communicate all patient issues and concerns to appropriate transplant physicians.
Direct and participate in all patient and family education.
Assist as appropriate in the evaluation and selection of potential living donors.  Serve as a liaison between the transplant program and the living donor primary care providers.
Manage and maintain appropriate patient monitoring system throughout work-up and while on donor wait list.
Maintain communication with patients’ referring physicians.
Assist in the procurement process by taking organ offer calls, dispatching the transplant team and arranging for potential organ recipients to be admitted to the hospital.
Prepare patients for discharge and outpatient follow-up.
Maintain and complete all data submissions as required by UNOS, JCAHO and/or other agencies.
Demonstrate skills and knowledge necessary to provide care appropriate to adult and geriatric patients, including knowledge of growth and development, the ability to obtain and interpret information to identify patient needs, and provide the care needed.
Perform assigned work safely, adhering to established departmental safety rules and practices.  Report to appropriate individual in a timely manner, any unsafe activities, conditions, hazards or safety violations that may cause injury to oneself, other employees, patients or visitors.
Performs other related duties as required.

Work Hours
Monday to Friday (8am to 4:30pm)
Required to be on call at nights and weekends
Education and Experience Required
B.S./BSN in Nursing required, MSN preferred. Critical care experience preferred.
Minimum of five years of related experience in health care, organ transplant experience preferred.
