Percent Predicted FVC:	%	Current PCO₂ is the value with a test date closest to todays date. This calculator uses the arterial blood gas test for the current PCO₂ value. If using the central venous test value, subtract six (6) mm Hg from the central venous value and enter that number. Enter a capillary test value as is. This calculator uses the arterial blood gas test for the Highest PCO₂ value. If using the central venous test value, subtract six (6) mm Hg from the central venous value and enter that number. A capillary test value is considered the same as an arterial test value. This calculator uses the arterial blood gas test for the Lowest PCO₂ value. If using the venous test value, subtract six (6) mm Hg from the venous value and enter that number. A capillary test value is considered the same as an arterial test value. To generate a score greater than zero, information about a New York Heart Association Class and Assisted Ventilation must be indicated on the form.

If specific values about heart catheterization (Pulmonary Artery Systolic Pressure, Mean Pulmonary Artery Pressure and Pulmonary Capillary Wedge Mean) are not indicated on the form or are lower than normal clinical values, normal clinical values will be applied.

Lung allocation scores apply to lung transplant candidates age 12 and older. If a date of birth entered on this form results in an age less than 12, the lung allocation score may be used for projected estimation purposes only.
